Reporting health and safety incidents

This page sets why and how we expect our contracted suppliers to report notifiable events, threats and other risks to health and safety to us.

We’ve put a reporting procedure in place so together we can meet our responsibilities under the Health and Safety at Work Act 2015 (HSWA) to protect people’s health and safety.

Notifiable events

A notifiable event is when any of the following occurs as a result of work:

    • a death

    • notifiable illness or injury

    • a notifiable incident

WorkSafe must be advised as soon as possible of any notifiable event, either by phone 0800 030 040 or using the form on the WorkSafe website.

If the notifiable event occurred as a result of work being undertaken for us, then you must also notify us as soon as possible using our online form.

Threats and other risks to health and safety

You also need to report to us any threats or other risks to health and safety that occur as a result of work undertaken for us.

How to report to us

Please report to us in writing using the online form as soon as possible, after reporting any notifiable events to WorkSafe.

Investigating the incident

It is your responsibility to investigate incidents. WorkSafe’s accident investigation form can be used if you do not have your own investigation forms.

A case owner, supplier manager or contract manager may contact you to follow up on any investigation undertaken and discuss actions to be taken by us to prevent similar incidents occurring.
