Contract transition

This service helps clients return to independence or maximise their independence following life changing injuries.

About HCS

The Home and Community Support contract increases flexibility in the types of services we support, and the number of suppliers we are partnered with.

We’re now working with a group of suppliers, some who offer specialist services, such as including Kaupapa Māori health. Having a wider range of providers will better reflect the communities we serve.

Our new service packages

Home and Community Support now includes two distinct services:

1. Return to Independence

Home and Community Support - Return to Independence (HCSRTI) provides personal care, home care, childcare and rehabilitation for kiritaki (clients) with injury-related needs.

The goal of HCSRTI is to enable kiritaki to regain their independence and quality of life in the home through a range of services.

2. Maximise Independence

Home and Community Support - Maximise Independence (HCSMI) aims to provide kiritaki (clients) with greater choice and control over home and community support, providing specialised supports for kiritaki following life changing impacts of an injury.

HCSMI connects kiritaki with support workers who have the skills and training to manage highly complex and individualised needs. The provision of personalised care and a stable team of support workers provides trust and confidence that their cares will be delivered maintaining their mana and ensuring they remain safe in their home and in the community.

HCSMI supports kiritaki through the following services:

  • Standard support
  • Complex support

In addition, our kiritaki can receive supports from:

  • Allied Health
  • Nursing Treatment
  • Childcare

This is a fee for service contract and all care hours will be approved by us following an independent assessment.
