Important: ACC32 Treatment Extension Form successfully moved to ProviderHub
We’ve moved the ACC32 webform from the website onto ProviderHub – our new online service. The ACC32 webform was discontinued as of 28 November.
If you need to complete an ACC32, you can submit the form through your Practice Management System (PMS) or through ProviderHub.
If your business can’t send an ACC32 through your PMS and has yet to sign up for ProviderHub, please contact ACCProviderHub@acc.co.nz.
Allied health providers working for Health New Zealand will have received internal communications about their options.
When using a PMS
Some PMS offer a service enabling you to complete and send an ACC32 form within the system. Talk to your PMS helpdesk if you’re not sure what your PMS offers. If your PMS doesn’t offer this task, you’ll need to join ProviderHub.
When using ProviderHub
Onboarding to ProviderHub starts with an email invitation we send to a business’s authorised person (usually the owner) to nominate a ProviderHub administrator. Once your administrator is onboard, then all other users can join. The administrator also approves access to the ACC32 form.
We’ve already sent an onboarding email to all impacted businesses. The 4,000 businesses already using ProviderHub report having a positive experience.
Onboarding is even quicker if you have an existing RealMe or My Health Account Workforce (MHAW) account, which you can use to login.
Onboarding to ProviderHub – what you need to do
- If you’ve received an email from us with the subject line: It’s time to nominate your ProviderHub administrator, please follow the instructions in the email to nominate your ProviderHub administrator and onboard.
- If you’re the business owner (or the person authorised to make decisions on behalf of the business) and you cannot locate our ProviderHub onboarding email, contact ACCProviderHub@acc.co.nz or call us on 0800 222 070 to request another email to be sent. Please include your ACC vendor number.
- If you’re an employee of a larger business and have not been asked to become a ProviderHub user by one of your colleagues, talk to the business owner/your manager about this email and whether you need to onboard to ProviderHub.
- Not able to onboard to ProviderHub promptly? Your business owner or authorised person may be uncontactable, for example, travelling overseas. Contact ACCProviderHub@acc.co.nz or call us on 0800 222 070. Please include your ACC vendor number.
- If you work with or for Health New Zealand/Te Whatu Ora, please wait for communication from them about our planned approach for you.
