Updated Surgical Assessment Report and Treatment Plan (ARTP) form now available
We’ve updated the Surgical Assessment Report and Treatment Plan (ARTP) form to make it clearer and easier to use.
This form is used to request approval for an elective survey procedure.
The updates were tested in a pilot programme last year. The aim of this trial was to understand the challenges faced with the previous version and to test the requirements of a digitised form.
We’ve updated the layout, changed mandatory fields, drop-down fields, clarification of the medical insurance statement and revised the clinical and proposed management/prognosis sections.
It’s important that you start using the updated ARTP form now. The previous version will no longer be accepted after 7 August 2025.
We’ve contacted Patient Management System (PMS) vendors to update their PMS e-forms. If you don’t use a PMS system, download a copy of the new form from our website.
