Use updated ARTP form for elective surgery requests
We want to remind you to use our updated Surgical Assessment Report and Treatment Plan (ARTP) form to request approval for an elective surgery procedure. After 7 August 2025, we’ll no longer accept the old ARTP form for these requests.
The form was updated in May 2025 to make it clearer and easier to use.
We’ve contacted Patient Management System (PMS) vendors to update their PMS e-forms. If you don’t use a PMS system, download a copy of the new form from our website:
