Improvements coming to ACC18 Medical Certification form
We're making improvements to the ACC18 Medical Certification form to make it easier to complete and to better support recovery planning for injured kiritaki (clients).
We’re working alongside your Practice Management System (PMS) vendors to update the ACC18 form and expect the updates to be phased in over a few months, starting in October 2026.
Why we're making changes
We've heard your feedback that the ACC18 form can be challenging to complete and can make it difficult to support effective recovery planning. We’ve listened to your suggestions and will be improving this form to make it simpler to use.
What you can expect
The updated ACC18 Medical Certification form will include clearer fields, improved functionality, and greater consistency with the ACC45 Injury Claim form. The changes will make it easier to:
- record a client's ability to work, including when they can undertake selected duties rather than being fully off work
- identify rehabilitation and return-to-work support needs
- provide additional recovery information where relevant
- communicate with ACC more effectively through better structured information fields.
We’ll update the ACC18 paper form to align with some of these changes, along with general updates to the branding and information included on the form.
We’re also improving the ACC45 Injury Claim form and will update you on these changes soon.
How to prepare
You don’t need to do anything right now. We'll share more information about the specific form changes closer to rollout, including guidance and supporting resources. We’re working directly with your PMS vendor, so you don’t need to contact them.
