ACC45 Injury Claim form enhancements coming to ProviderHub
If you use ProviderHub, you’ll see changes to the ACC45 Injury Claim form from November.
The updates will make it easier to share information you are often already capturing, helping to reduce follow-up requests, improve information quality and support timely decisions. We’ll let you know when the changes are live and share a user guide to support you.
Why we're making changes
We've heard consistent feedback from you that the ACC45 can make it difficult to support effective recovery planning. We've listened to your suggestions to improve these forms, making them simpler to use and easier to complete in practice.
What’s changing in the ACC45 Injury Claim form
- Improved rehabilitation and assistance requests: If you think rehabilitation or other support is needed, you’ll be able to provide more specific details about the request.
- New disability questions: Two optional questions to help us to better understand the access, experience, and outcomes of disabled people. This information will help address a current gap in our administrative disability data, support more robust reporting, and enable us to better identify and respond to the needs of disabled clients.
- Consultation type recorded: You’ll be able to indicate whether the consultation was in person or by telehealth.
- Clinical notes can be attached: You’ll have the option to attach clinical notes that are relevant to the injury when submitting the form. This may reduce follow-up requests, speed up decisions, and reduce delays for clients. You can attach clinical notes when submitting the form, or save your claim lodgement form as a draft and add them when you're ready to submit.
- ACC42 dental claims: Dental providers will be able to submit dental injury claims through the existing Claim Lodgement (ACC45) feature in ProviderHub.
For certifying providers
There’s a new return-to-work support section allowing you to easily capture requests for support to help the client return to work. This also ensures better alignment between the ACC45 form and ACC18 Medical Certificate.
