Improvements coming to the ACC45 form
We’re enhancing the ACC45 Injury Claim form in response to your feedback. The changes aim to reduce administrative burden, improve data quality, make certification easier, and better support recovery planning.
The form updates will be phased in over the coming months, starting in October 2026. You’ll hear more from us and your Practice Management System (or other system) provider when the updates are ready.
What you can expect
You’ll see a range of practical improvements to the ACC45, including clearer fields that enable better support for recovery and return-to-work planning, improved rehabilitation and assistance requests, and greater flexibility to provide additional information where relevant.
Why we're making changes
We've heard consistent feedback from you that the ACC45 can be challenging to complete and can make it difficult to support effective recovery planning. We’ve listened to your suggestions to improve this form, making it simpler to use and easier to complete in practice.
ACC45 improvements
- New return-to-work support section: A dedicated section so you can easily capture requests for support to help the client return to work. This also ensures alignment between the ACC45 form and ACC18.
- Easier to record changes in work capacity: This will allow you to capture different levels of work capacity on a single certificate. Previously this would have required you to complete the ACC45 and ACC18 in the same consultation.
- New disability questions: Two optional questions to help us to better understand the access, experience and outcomes of disabled people. This information will help address a current gap in our administrative disability data, support more robust reporting, and enable us to better identify and respond to the needs of disabled clients.
- Improved rehabilitation and assistance requests: If a provider believes rehabilitation or other support is needed, they'll be able to provide more specific details about the request.
- Nurse Practitioners can certify more easily: A new provider type will allow Nurse Practitioners to submit the certification/incapacity section of the ACC45, avoiding the need for additional workarounds.
- Consultation type recorded: You’ll be able to indicate whether the consultation was in person or by telehealth.
- Clinical notes can be attached: You’ll be able to attach clinical notes when submitting the ACC45. This change makes it easier for you to share relevant clinical information with ACC, reducing administration time and supporting timely decisions that help patients progress in their recovery.
How to prepare
There's nothing you need to do at this stage. You’ll hear more from us and your PMS (or other system) provider as the changes are rolled out to help ensure a smooth transition.
