You can use this confidential online form to tell us about any activities that concern you, relating to:

  • an ACC client or,
  • an ACC provider.If you have concerns about a different matter (eg. ACC levy) please close this form and email us instead.

    We hold this information you submit in confidence. It will only be used as part of our fraud investigation. When you submit this form we'll assess the information provided and determine next steps.
