Update on the proposed clinically-led secondary care model

Released 03/11/2026

In response to feedback from the sector on the design of a new clinically-led secondary care model for musculoskeletal injuries, we’re looking to test the proposed model through a pilot in a small number of regions, subject to Board approval.

A consistent theme from our engagement with suppliers, professional bodies, and the Musculoskeletal Secondary Care Clinical Advisory Group has been the importance of testing a change of this scale before making decisions about wider implementation.

If approved, the pilot would allow us to test the model in selected regions, understand how it works in practice, and gather evidence about its impacts on clients, providers, and the wider health sector.

It would also help us assess whether the model achieves its intended outcomes and identify any changes needed to support future decisions about the model.

The proposed model aims to provide a more consistent, transparent, and coordinated experience for clients, helping them access the right care at the right time and supporting better recovery outcomes. It would retain all services currently delivered through the Clinical Services contract, along with the addition of some new components. As a result, the Clinical Services contract would be divided into two contracts – one to provide services for people with musculoskeletal injuries and one to support people with non-musculoskeletal injuries.

For musculoskeletal injuries, the proposed model would be delivered through multidisciplinary care networks. Key features would include stronger clinical governance, specialist assessment and treatment capability, coordinated triage and assessment functions, and improved transparency and reporting.
