To complete and send an ACC32 you will need to use ProviderHub or your Practice Management System.

If you haven’t received an invitation to get set up on ProviderHub, please contact us. Please include your ACC vendor ID in your email.
PMS
On some PMS you can complete and send an ACC32 form. Talk to your PMS Help desk or your IT support. If your PMS doesn’t offer this task you will need to join ProviderHub.
